You’re sitting in the dental chair. The hygienist calls out a series of numbers: “Three, three, four, two, five.” A metal probe gently touches along the gumline. It doesn’t hurt exactly, but it’s a strange sensation. What just happened? That was a periodontal screening. And those numbers mean something important for your health.

The Short Version
A periodontal screening measures the space between each tooth and the gum tissue that surrounds it. We call these spaces “pockets.” Healthy pockets are shallow. Unhealthy pockets are deep.
Here at Holmes Family & Cosmetic Dentistry, we perform this screening on every adult patient during routine checkups. It takes about two minutes. And it helps us catch gum disease long before you notice anything wrong.
Why Those Numbers Matter
Healthy gums fit snugly around each tooth. A normal pocket depth is between one and three millimeters. When we call out a “two” or a “three,” that’s good news.
A “four” means the gums are starting to pull away. Bacteria can hide in that space. Your flossing and brushing may not reach deep enough to clean it properly.
A “five” or higher signals gum disease. The deeper the pocket, the more advanced the problem. Without treatment, the bone supporting your tooth can begin to break down.
The good news? We can reverse early stage gum disease, called gingivitis. For more advanced stages, called periodontitis, we can often manage and stop the disease with proper care.
Does It Hurt?
The probe we use for a periodontal screening is small and curved. It has a blunt tip designed to measure, not poke. Most of our patients describe the sensation as pressure, not pain.
If your gums are already inflamed from existing disease, the screening may feel more sensitive. That tenderness is actually a sign that your gums need attention. Healthy gums do not bleed or hurt when we measure them.

What Happens After the Numbers?
After the screening, we explain your results in plain language. No confusing medical terms. Just a clear picture of what is happening under your gums.
- If all your numbers are threes or lower, that is excellent. Keep doing what you are doing. We will see you for standard cleanings, and you keep up with daily brushing and flossing at home.
- If some numbers are fours, we may recommend a deeper cleaning, called scaling and root planing. This removes bacteria from below your gumline. Your gums can then heal and reattach to your teeth.
- If fives or higher show up, we create a personalized treatment plan just for you. Dr. Ryan Holmes or Dr. Phil Hoyt will discuss your options, which may include more frequent cleanings or additional treatment. Our goal is always to save your natural teeth and stop disease from progressing.
